What Happens Next?

If you arranged concerning an immediate need:

• Please call us at our Bellevue (425-641-6100) or Federal Way (253-874-9000) locations to report the death, so we can dispatch our care team.
• Notify the place of death that Cascade Memorial is the funeral home you have chosen and that you give them permission to release your loved one into our care.
• Please have the next of kin complete the following form:
  • Electronic/Online Authorization for Cremation and Disposition (Preferred Option)
  • Printable Authorization for Cremation and Disposition
• The death certificates will be available after the cremation has occurred. Click here for information on how to order death certificates.
• We will be posting a basic online obituary on www.cascadememorial.com. Please feel free to compose a biography/life story and a photo. Send it to info@cascadememorial.com and we will be glad to update it on our website for you.

If you are arranging in advance:

• Please notify your family, or those close to you, that Cascade Memorial will be handling your pre-paid final arrangements.
• Please complete the Electronic/Online Authorization for Cremation and Disposition
• Click here to print the Notice of Cancellation of Trust Form. You may use this form to cancel the trust within 30 days.
